Sample Mail-In Form

Although we encourage online giving as a more cost-effective and efficient way of giving, the following solicitation form can be included in your newsletter or linked to from your giving website for people who prefer to mail donations. The highlighted areas indicate variable text and in these locations you will need to include information specific to your department or program. Program or Department

The name of your program or department, not the name of the fund for which you are seeking donations.

Fund Description

This is a brief description of the purpose of your department or program.

Tracking Code

The tracking code will allow you to determine how many donors respond to this piece. To request one, contact LAS Advancement at lasgifts@illinois.edu. Please provide the answers to the following questions when you do: Approximately how many newsletters do you intend to mail? When will your newsletter be sent?

Fund Number

The number displayed here should be the UIF account number for your fund, not your banner account number. If you are unaware of your UIF account number please contact LAS Advancement at lasgifts@illinois.edu with your banner account number and the name of your fund.

Name of Fund

If you are not aware of the proper name for your fund, please contact LAS Advancement office lasgifts@illinois.edu with the UIF number or the banner number for your account.

URL

If your department or program does not have its own giving page, please use the standard LAS url: www.las.illinois.edu/giving. This site allows donors to specify where they wish to direct their gift.

Gift Message

This should be a brief statement indicating how your department spends the income from the fund for which you are soliciting.
